The Solution Rating System

With so many solutions, how can you tell which solutions are most likely to help you and which ones are not? To provide you with a tool to use, we rate our solutions based on various elements that most accurately determine if a solution is a quality solution. To explain what factors affect the solution rating, here are the elements we take into consideration when formulating our solution rating.

  • The Grade of the Solution
  • The Zone Rank of the Expert Providing the Solution
  • The Number of Author and Expert Comments
  • The Number of Experts Contributing
  • The Feedback of the Community

Your Input Matters
Because of the way the system is set up, the most important variable in this equation is you. Having trouble wording this: Working on an Healthcare DB for capturing Inpatient data for one or multi days

I'm doing a db for a hospital service that sees patients on an inpatient basis.  These patients may be seen once or 25 times (just once a day but for multiple days).  I didn't have that little bit of information  when I put the db together.  So, I have the data entry page attached that I originally did. How do make that into something where a visit can be entered multiple times w/o making 25 different data entry fields.  And again the pt may only have 1 consult (i.e. one date entered 9/10/08) or multiple entries for visits (i.e. 09/10/08; 09/11/08, 09/12/08.... and so on).  They will also be tracking that same data per visit, each time the patient is visited they will be assessed for the same information I have listed below.
Date of Visit
Type of Visit
Location (room # that the pt is in or Unit, the location may or may not change, could be moved to ICU )
PRS (1-10 Pain Rating Scale)
Non Pain Symptom (a symptom outside of pain (Shortness of Breath, Chest Pain...))
nPRS (1-10 Scale)
HURT (1-10 Scale)
Other Services (Family Meeting...)
Recommendations (RxO, RxP...)
I also don't know how to avoid having blank spaces in the db if the pt is only seen for 1 day and there are 25 or 30 blank data entry fields that didn't get filled in because the pt was only seen once.  I understand blanks can cause corruption.
