How to handle Paypal "Pending" payments

Hello,

I am about to implement PayPal payments on my website. I went through several documentation and figured out the theory, but before putting it into practice I have a few doubts about pending payments.

According to your experience, after how much time a payment which is considered pending (for various reasons) is confirmed by Paypal?

Also, what is considered the best practice in this case: to send a confirmation e-mail when the payment is confirmed?

Lastly, it is true that the final confirmation will be sent to the address specified as "notify_url", with the same parameters as the previously received "pending" message? In that case, I guess will have a server-side code to discriminate on the POST variables and act consequently.
